The Barbarian lands would host many free build cities. As would the Civilised cities have much diversity in which hopefully the people who are into building to restrictions, will build to restrictions happily, no matter the challenge. Also there is much to choose from hopefully giving it a longer life.
Personally I feel I have a good system grafted for the Moderation. All but the Barbarian Kingdoms will be whitelisted. In all honesty the councils of these Nation Cities will be very small at the beginning and comprise of a small amount of people who will just happily build away in regards to the laws of architecture. These people will undoubtedly respect the laws and will tell the Moderator of any injustice who will then give the go ahead to demolish it. I envisioned the Moderator to also choose the Presidents of the Nations who will then have the ability to divy land, delegate jobs and process land transactions. I don't see how hard it would be, just a plaque on the wall of your Main Civil building stating your the President and showing your council would be enough. If people have a problem with your rule, they can appeal to the mod for change. Idealistically the Nations should run by themselves happily and governments will take the course of the people who are in that country, but I always look on the bright side.

im not sure you can toggle certain regions for whitelist permissions without writing or dowloading a plugin.

Yeah I was thinking this, so the map would most likely work best if there was a 4 portals in the Barbarian lands that connected each realm. This would make travel a lot easier as well. This would also give the chance of the barbarian lands to be bigger - which it needs to be.

This makes it all nice and simple. Is there any reason why the portals are so far away in the Outlands?

The portals are so far away mainly to prevent grief, though that doesn't work out all that much.

Well, yeah, but the point being, wasn't oxytown originally not whitelisted? As well as mintetopia for those with no whitelist access.
