When you make a TNT cannon and launch yourself out of it instead of using TNT, and then smashing your face into the ground!

[Creeper]

This doesn't exactly fit like a kitten into the embarrassing theme, but..

You are walking around in beta, chopping down trees, and you turn around to see a creeper. The creeper explodes, which knocks you unconscious. You wake up hours later to find yourself strapped to a chair surrounded by wired TNT. Looking up, two creepers in black tuxedos stare back at you. They give out wispy laughs as they walk out of the room, hitting a lever.

And then creepers and all other mobs unite and take over the world whilst eating cake

Standing on diamond ore- proceeding to mine it and falling into lava, burining it and all your diamond equipment...
Then a creeper blows you up after you spawn in the middle of nowhere.
